Sonder Homecare is a Minnesota home and community based services provider licensed under DHS chapter 245D. We don't run a facility. Our caregivers go where our clients already live: their own homes, a family member's home, and the community around them.
Each client has a person-centered plan built around their own goals and routines, and the same caregivers show up week after week. Families and case managers get updates without having to ask for them. Our office is in Bloomington; our work happens at your address.

For Families
Inviting a caregiver into your home is a big decision. Meet ours before services start, and keep the routines that already work for your family.

For Case Managers

What we provide.
Basic and intensive 245D services, delivered in your home and community.
Our most-referred service. One-on-one support in the client's home and community: community participation, health and safety, household management, and adaptive skills. The with-training versions build skills against measured outcomes, and family training coaches the people who live alongside the client.

Short-term care that gives family caregivers a real break, provided in the client's home or in another approved setting outside it. Planned ahead or arranged when a family suddenly needs coverage.

Overnight staff in the client's own home. One of the services we're asked for most, and one of the hardest for families to cover alone. Trained caregivers stay on site so clients are safe and families can sleep.

Help with the work that keeps a household running: cleaning, laundry, meals, and general home management, with assistance for clients who want to do these tasks themselves.
